Marian Punsalan, RN, Director of Nursing Services, has been a Registered Nurse for more than 30 years. She started her career working acute care at hospitals and chose to transition to a skilled nursing facilities about 20 years ago to fulfill her desire to care for and have the opportunity to know her patients. We are so happy to have had Marian at Mesa Glen Care Center for the past 7 plus years. Relying on her nursing skills, Marian assesses and treats the physical and emotional needs of each resident, but it is her kind and gentle nature that gives her the compassion and thoughtfulness that gives our residents the satisfaction of knowing that she truly cares about them. Her insight into the wants and needs of each resident allows her to direct her staff to provide care and services for each resident’s specific needs.

Marian is responsible for the entire nursing department, their duties, and performance. She hand selects her Registered and Licensed nursing personnel and works very closely with our Executive Director in the coordination of services provided by our facility in support of our nursing department. Marian is rarely in her office, preferring to spend her time at one of our two nursing stations or in resident rooms.

Lezlie Hicks, LVN, Director of Staff Development, is a dedicated professional with over 20 years of nursing experience. Lezlie has been with our company for over 8 years performing the tasks of MDS, DSD and completing her AIT program. Lezlie is an integral part of the facility’s delivery of care and service with integrity. She works closely with our Director of Nursing Services and our Executive Director in addressing and identifying concerns and providing excellent follow through to ensure that the traning supplied to our staff is effective in meeting the needs of our residents and staff. Lezlie has been a Licensed Vocational Nurse for many years and is committed to help those in need. Lezlie ensures that each resident’s stay at Mesa Glen Care Center is an enjoyable experience.

Lezlie is also responsible for the services and care provided by our Certified Nursing Assistants and our Restorative Nursing Assistants. She provides in-service training to all departments on care issues, fire and disaster policies, and resident’s rights. She provides upwards of 6 in-services per month to our staff covering each of our three work shifts. Lezlie is responsible for the hiring of our Certified Nursing Assistants and conducting background checks on all staff being considered for employment.


Jocelynn Magana, LVN, MDS Coordinator, has been employed at Mesa Glen Care Center for three wonderful years. She assists with the development of each resident’s individual care plan working with our licensed nursing staff, our Certified Nursing Assistants, our activity staff, and our dietary staff in learning the habits, concerns, deficits, and abilities of each resident. Jocelynn is a knowledgable nurse with an easy going manner, which makes her easy to talk to. Families and residents often feel very comfortable sharing with Jocelynn their problems, concerns, and desires. Jocelynn is able to use this information to truly personalize each resident’s plan of care.

Jocelynn heads our care planning committee, which includes our Dietary Service Supervisor, our Social Service Designee, and our Activity Director. The resident’s plan of care assists our staff in knowing what needs to be done for a resident and what abilities we need to encourage. Jocelynn also schedules care plan meetings with residents and their families or responsible party. Care plan meetings are scheduled within 21 days of admission and each quarter after admission. We also conduct care plan meetings when a resident has a decline or an improvement in their physical or psychosocial condition. The purpose of care plan meetings is to review the needs of the resident with the resident, and their family or responsible party. This review helps us ensure that we are addressing all needs.
